Título

Blood under the bridge: imaging aspects of coronary stenting and myocardial revascularization

Introdução e objetivo(s)

Coronary artery disease is one of the leading causes of death worldwide, and management through coronary stenting and vascular bypass grafts are well-established techniques. This educational exhibit aims to demonstrate both common and uncommon cases of stenting and myocardial revascularization, with clinical-radiologic correlation.

Método(s)

Literature review and retrospective analysis of cases of coronary stents and myocardial revascularization, obtained through coronary computed tomography angiography (CTA), coronary angiography, intravascular ultrasound, and optical coherence tomography from the authors' experience and/or their own files. The imaging characteristics will be demonstrated using anatomical sections, multiplanar reconstructions, volumetric acquisition, and simplified pictorial schemes, sometimes complemented by other imaging methods.

Discussão

Recent advancements in cardiothoracic imaging have improved the detection and prognostication of cardiothoracic disease, resulting in more accurate diagnoses and better patient management. Although vascular bypass grafts are the technique of choice, particularly in cases of diffuse or multivessel disease, coronary stenting has become a mainstay in coronary revascularization therapy. Coronary CTA has evolved as a valuable tool in the diagnostic workup of patients after coronary revascularization therapy. It has a high negative predictive value for ruling out significant vascular stenosis.

Conclusões

Coronary CTA imaging of coronary artery stents has evolved into a reliable tool in the diagnostic workup of patients after coronary revascularization therapy. The development and evaluation of comprehensive examination protocols that assess both morphology and hemodynamic significance will further enhance the diagnostic potential of coronary CT.
