Revealed Mysteries: Strategies for the Diagnosis and Management of Early and Late Fetal Growth Restriction

The diagnosis and management of early and late-onset fetal growth restriction are essential for fetal health. This study aims to discuss the importance of early detection and the distinction between small for gestational age (SGA) and fetal growth restriction (FGR), highlighting the impact of this differentiation in clinical practice. Additionally, it addresses strategies for ultrasonographic evaluation, including essential parameters, surveillance frequency, and interpretation of fetal growth charts guided by ISUOG criteria. Finally, it discusses clinical management based on the severity of SGA/FGR and gestational age, as well as the ideal timing and mode of delivery, while presenting a perspective on future research in the field.

Método(s)

A literature review and analysis of clinical protocols related to the diagnosis and management of SGA and FGR were conducted, including surveillance criteria based on ultrasonographic evaluation according to ISUOG guidelines and clinical management strategies based on the severity of the condition and gestational age. The impact of timing and mode of delivery on perinatal outcomes was also analyzed, considering current best practices.

Discussão

The use of ultrasound, following parameters such as estimated fetal weight, amniotic fluid index, and Doppler, combined with the interpretation of fetal growth charts according to ISUOG, enables precise and personalized surveillance. Clinical management should be guided by the severity of the condition and gestational age, considering strategies to prolong pregnancy whenever safe and determining the ideal timing for delivery. Furthermore, ongoing research is expanding knowledge about the underlying mechanisms of SGA and FGR, fostering advancements in diagnostic and therapeutic approaches.

Conclusões

The management of fetal growth restriction requires a multidisciplinary approach, the use of ISUOG criteria, and strategies tailored to the severity and gestational age. Continuous advancements in the field aim to optimize outcomes and emphasize the importance of professional development.
