Unlocking the Future of FFRCT: The Role of AI in Transforming Cardiac Imaging
Introdução e objetivo(s)
The objective of this study is to present challenging cases that illustrate the role of fractional flow reserve by computed tomography (FFRCT) in complex diagnostic scenarios, such as coronary stents, myocardial bridges, and anomalous pathways. Additionally, it aims to discuss the appropriateness criteria and interpretation recommendations for FFRCT, highlighting its relevance in clinical decision-making. The study also defines software concepts related to FFRCT, such as those based on Computational Fluid Dynamics (CFD-FFRCT) and Deep Learning (DL-FFRCT), exploring the available solutions, their advantages, and disadvantages. Finally, it evaluates the impact of artificial intelligence on software development, focusing on its contribution to greater diagnostic accuracy and faster clinical outcomes.

Different types of software based on CFD-FFRCT and Deep Learning were compared, highlighting their practical applications, advantages, limitations, and impact on clinical practice. The discussion also included the appropriateness criteria for FFRCT and interpretative recommendations, integrating the impact of artificial intelligence in enhancing these tools.
Discussão
FFRCT has proven to be an essential tool in complex scenarios, such as the evaluation of coronary stents, identification of myocardial bridges, and anomalous pathways. The study highlighted that CFD-FFRCT-based software offers high precision but with greater computational demand, while DL-FFRCT-based software delivers rapid results and high accuracy, enabling swift clinical decision-making. Artificial intelligence has played a crucial role in the development of these tools, allowing greater integration with clinical workflows, faster results, and increased diagnostic reliability. The appropriateness and interpretation criteria for FFRCT were also emphasized as fundamental for its correct application in clinical practice.
Conclusões
FFRCT is a powerful tool for functional assessment in challenging cases of coronary artery disease, especially with the support of advanced technologies such as artificial intelligence and deep learning. The choice of the most suitable software depends on specific clinical needs, balancing precision, speed, and availability. The continuous evolution of these technologies, driven by artificial intelligence, is transforming diagnostic approaches and clinical management, promoting greater efficiency and accuracy in patient care.
