PSEUDO-UNICORN UTERUS AS COMPLICATION AFTER UTERINE LEIOMYOMA EMBOLIZATION: CASE REPORT

The objective of this work is to present a case that represents a late complication of uterine artery embolization.
História clínica
Patient, 38 years old, with pelvic pain and abnormal uterine bleeding, performed transvaginal ultrasonography and pelvic magnetic resonance imaging (MRI) that identified a myometrial nodule consistent with myoma. After one year of clinical treatment and refractoriness of symptoms, the assisting medical team decided to perform myoma embolization. In the post-procedure follow-up exams, synechia of the left uterine horn was identified on magnetic resonance imaging, ultrasound and hysterosalpingography, at the site of the embolized myoma, characterizing a pseudo-unicornuate uterus resulting from a complication of previous embolization.
Discussão e diagnóstico
Although uterine artery embolization is a safe and effective uterine-sparing treatment option for the treatment of symptomatic leiomyomas, complications can occur. The most frequent early complications are uterine ischemia, infection, intracavitary hemorrhage and pulmonary embolism. The most prevalent late complications are amenorrhea spectrum findings, premature ovarian failure, subsequent pregnancy-related complications (such as uterine synechiae). MRI has been proved useful in assessing the success of UEA, as well as its potential complications.
Conclusões
EMBOLIZATION OF UTERINE FIBROIDS IS A THERAPEUTIC TOOL THAT HAS BEEN WIDELY USED, THUS, RADIOLOGISTS MUST KNOW HOW TO EVALUATE POST EMBOLIZATION FINDINGS AND RECOGNIZE EARLY AND LATE COMPLICATIONS.
