RADIAL SCAR IN THE LIGHT OF ACR BI-RADS®

Introdução e objetivo(s)

Although the benefits of screening programs for breast cancer are well known, there are concerns regarding the possibility of unnecessary biopsies and surgeries caused by imaging findings in patients who do not have breast cancer, caused by benign lesions that may present as suspicious findings in imaging exams. In fact, up to 14% of image-guided biopsies performed due to suspicious mammograms yield high-risk breast lesions, such as radial scars/complex sclerosing lesions (RS/CSL). The objective of this presentation is to provide a didactic demonstration of the appearance of RS/CSL in mammography, tomosynthesis, ultrasound, magnetic resonance imaging and its histological correlation. Moreover, it aims to present current evidence that supports conservative management in cases where radiological and histological findings are concordant, in favor of clinical and imaging follow-up.

Método(s)

Case-based didactic review of the findings of radial scar/complex sclerosing lesion in different imaging modalities (mammography, tomosynthesis, ultrasound, magnetic resonance imaging) based on our service's digital archive and correlated with histological findings, as well as a literature review.

Discussão

The release of the 6th edition of the Breast Imaging Reporting & Data System (BI-RADS ®) is scheduled for 2024. In this edition, some of the changes possibly include the exclusion of radial scars/complex sclerosing lesions from high-risk lesions group, which will now probably be denominated "benign lesions with upgrade potential".

Conclusões

Radial scar is a lesion that can simulate breast cancer, however, it is benign and current evidence supports conservative management in most cases. Consequently, it is important for the radiologist to understand its pathology, imaging characteristics and radiological-pathological correlations.
