INSIDE THE SPINAL CANAL: A JOURNEY THROUGH INTRASPINAL PATHOLOGIES

Introdução e objetivo(s)

The main objective of this exhibit are (1) review the anatomy and main pathologies affecting the medullary canal of the cervical, thoracic and lumbar spines; (2) create a diagnostic flowchart with information about what neurosurgeons need to know, (3) describe the main imaging characteristics used to narrow down differentials.

Método(s)

This pictorial essay aims to discuss various pathologies through the demonstration of illustrative cases such as (1) mixopapillary ependymoma, (2) intradural spinal lipoma, (3) chordoma, (4) schwannoma, (5) plexiform neurofibroma, (6) lipoma of the filum terminale, (7) metastases, (8) vertebral body hemangioma with epidural component, (9) ependymoma, (10) astrocytoma, (11) hemangioblastoma, (12) meningioma and (13) dermoid cyst.

Discussão

Lesions within the spinal canal can range from benign to highly malignant, with only 20% of these lesions being intramedullary. Other possible origins include nerves, meninges, and vessels.
Ependymomas and astrocytomas constitute a majority of spinal cord tumors, with ependymoma occurring predominantly in adults and astrocytoma occurring predominantly in children.
Back pain is the most common symptom (67%), it is generally of a diffuse nature and worsens when the patient is in a horizontal position.

Conclusões

Overall, this exhibit provides a comprehensive overview of intraspinal lesions and their characteristics, contributing to accurate diagnosis and treatment planning.
