Lung Cysts: Practical approach to differential diagnosis

The purpose of this exhibit is:
1. Review of the main causes of lung cysts and their, imaging aspects, clinical features and pathophysiology.
2. To illustrate the main imaging findings and practical points in order to address the finding of multiple lung cysts in chest CT scans.

1. Introduction: description of lung cysts, epidemiology, and main diseases related.
2. Section-structured overview on how to differentiate lung cysts by their tomographic
presentation and for each disease describe imaging aspects, etiology, pathophysiology, and
epidemiology:
a) Multiple and diffuse. The differential diagnosis includes lymphangioleiomyomatosis
(LAM) and Langerhans cell histiocytosis, and rarer, constrictive bronchiolitis
b) Scattered: Lymphocytic interstitial pneumonia (LIP), Follicular bronchiolitis, Birt-Hogg-
Dubé syndrome, Light Chain Deposition Disease, Infectious: Pneumocystis jiroveci
pneumonia, pneumatocele.
c) Associated with interstitial lung disease: Hypersensitivity pneumonitis, Giant cell
Pneumonia, Descamative Interstitial Pneumonia, Scleroderma (NSIP) and Amyloidosis
d) Malignancy: cystic (cavitated) and treated metastasis.

Conclusões

Lung cysts are a common entity and part of a radiologist's daily routine. Knowing how to differentiate the main tomographic patterns helps guide accurate clinical diagnosis, which is often part of a multidisciplinary discussion.
